Please complete and submit this application form by July 1, 2017. You will be notified of acceptance or rejection by the middle of July. The accepted applicants will then need to submit a non-refundable deposit of 200 USD through the Lesson Study Alliance website.

Suggested lengths are given for some of the responses, but these are approximate. Your response may be longer or shorter.
